North Country Learning Center (NCLC) is a New Hampshire Department of Education approved year-round school offering services to students with special needs for ages 8 - 21. North Country Learning Center is preposed to assist students with transitions from their local districts and back to those districts.
We know parents often have concerns about services and support for their children with special needs after the age of twenty-one.
Through North Country Independent Living (NCIL) alternatives for adult transition designed to each individual based upon strengths, abilities, and goals of the individual. An initial assessment is made to determine where the person will find the most success in reaching the goal of maximum independence.
Alternatives are available with minimal support, 24/7 staff support or home-based. Supported residential care is available in a shared living arrangement with peers and 24/7 staff support. Supported apartment living provides the individual with an opportunity to move back into a desired independent lifestyle with minimal supports. A Plan of Care is established to determine direct care services needed. Home based services is a viable alternative for suitable individuals. These services may include cognitive rehabilitation, speech services, occupational therapy, physical therapy, personal care, employment services, community integration, counseling services and case management.
